- Delivering social work interventions across all ages, including assessments, advocacy, referrals, group work, case coordination, and long-term planning.
- Collaborating with multidisciplinary teams and healthcare providers through conferences and meetings.
- Assessing patient and family needs, communicating effectively, and adjusting care plans based on ongoing evaluations.
- Minimum one year of social work experience or waived for current employees.
- Masters degree in Behavioral Science or Social Work.
- Licensure as an Independent Clinical Social Worker, Mental Health Counselor, Advanced Social Worker, or Marriage and Family Therapist in Washington.
- Excellent communication skills, team collaboration, and commitment to diversity.
- Preferred: two years post-masters experience, care coordination in various settings, knowledge of case management, conflict resolution, team building, and proficiency with Microsoft Office and electronic charting.
