Role: Hospice LPN Float

Responsibilities

  • Provide practical nursing care in patients’ homes
  • Monitor changes in patient conditions
  • Administer medications and treatments
  • Assist with symptom and pain management
  • Document care accurately
  • Coordinate with hospice nurses and interdisciplinary team members
  • Communicate updates to patients and caregivers
