- Building and maintaining relationships with hospitals, physicians, case managers, and community partners
- Generating referrals through account management and facility presence
- Coordinating admissions based on clinical appropriateness
- Serving as a clinical resource for referral sources and families
- Conducting outreach activities to support census growth
- Participating in care transition planning and interdisciplinary communication
- Providing excellent customer service and tracking referral activity
- Supporting evening and weekend admission calls as needed
- Ensuring compliance with regulations and policies
- Current LPN or RN license in good standing
- Clinical assessment, communication, and relationship-building skills
- Organizational and time management abilities
- Valid driver’s license and reliable transportation
- Proficiency with Microsoft Office and computer systems
- Skilled Nursing Facility (SNF) settings
- Admissions or clinical liaison roles
- Healthcare sales or business development background
