Key Responsibilities
- Travel between patient homes to provide ADL support and companionship
- Collaborate with clinical teams and communicate changes in condition
- Administer basic first aid and assist with treatments under supervision
- Ensure patient comfort and cleanliness
- Support coordination with family and other care professionals
- CNA license in Connecticut
- Prior experience in senior care or home care preferred
- Ability to adapt to various environments and schedules
- Strong communication and organizational skills
- Reliable vehicle and valid driver’s license
