MDS Coordinator (Registered Nurse/RN)

The RN MDS Coordinator is responsible for coordinating and assisting with the accurate and timely completion of interdisciplinary MDS Assessments, CAAs, and Care Plans in compliance with CMS RAI Manual Regulations, laws, and standards.

Qualifications Include

  • Associate’s or bachelor’s degree in nursing from an accredited institution
  • Active RN license in applicable state
  • Minimum of two years nursing experience, with geriatric nursing preferred
  • CPR certification, maintained throughout employment

Key Responsibilities

  • Coordinate assessment processes and ensure compliance with regulations
  • Report patient condition changes and educate staff on documentation updates
  • Review care plans, validation reports, and CMS submissions to prevent delays and errors
  • Participate in interdisciplinary meetings and perform staff nurse duties as needed
  • Maintain confidentiality, adhere to policies, and demonstrate professionalism and integrity
  • Provide excellent customer service, assist with emergencies, and support team communication
