Role: Licensed Practical Nurse (LPN) providing patient-centered care in a primary care setting.
Responsibilities: Deliver nursing services including patient assessments, education on preventive care and chronic disease management, medication administration, and assisting providers with procedures and tests.
Prepare patients for visits, collect specimens, perform point-of-care testing, and manage wound care.
Coordinate care, assist with referrals, and maintain a safe, clean clinical environment.
Engage in performance improvement and patient outreach activities, serve as a patient experience advocate, and handle confidential information.
Support front office duties and participate in on-call responsibilities as needed.
Qualifications & Skills: Completion of an LPN program, current PA licensure, CPR certification, and 1-2 years of primary or specialty care nursing experience, including triage or emergency care.
Strong clinical judgment, communication skills, attention to detail, and ability to work collaboratively.
