Role: Care Coordinator for the Birth to Three Program, a federally mandated Early Intervention service supporting children ages 0–3 with developmental delays or disabilities and their families.
Responsibilities: Partner with families, service providers, and community resources to promote child development through conducting assessments, creating and implementing individualized, family-centered service plans, coordinating services, providing education and advocacy, maintaining accurate documentation, facilitating team meetings, and collaborating with internal and external stakeholders.
Qualifications: Bachelor's degree in Social Work, Child Development, or related human services field required; one year of supervised experience with families with special needs preferred. Valid driver's license, reliable transportation, strong organizational and communication skills, and ability to work in diverse settings are essential.
Skills & Attributes: Experience working in community-based environments, crisis response capability, adaptability to travel, and physical activity including home visits.
