WORK SCHEDULE: 40 hours / wk (four 10-hour days).

OTHER:

We are currently seeking a highly skilled Family Nurse Practitioner (FNP) or Physician Assistant (PA) to join our team of healthcare professionals. SUMMARY:
Responsible for providing family medical services, including assessing and managing patients in the clinic and other locations (assisted living facilities, patient homes, etc.), within the scope of licensing, training, and credentialing.

As a Family Practice FNP / PA, you will work closely with our team of physicians, FNPs, PAs, medical assistants, and nurses, to deliver high-quality medical care across the lifespan. You will assess, diagnose, and treat a wide range of health conditions, develop care plans, and educate patients about disease prevention and healthy lifestyle choices.

RESPONSIBILITIES AND DUTIES:
Conduct comprehensive patient assessments and physical exams.

Diagnose and manage acute and chronic medical conditions.

Develop individualized treatment plans and provide appropriate follow-up care.

Prescribe medications, therapies, and other interventions as needed.

Provide patient education on disease prevention, wellness, and treatment options.

Collaborate with physicians and other healthcare professionals to ensure the highest level of care.

Maintain accurate and up-to-date medical records in compliance with regulatory standards.

Participate in community outreach and wellness programs.

QUALIFICATIONS:
Education/Certification:
Master’s degree in Nursing or Physician Assistant program

Active certification as a Family Nurse Practitioner (FNP) or Physician Assistant (PA-C)

Current state licensure in [State]

Current DEA license

Current BLS/ACLS certification

Experience:
Minimum of 2 years of experience in a primary care setting preferred

Experience with pediatric, adult, and geriatric care is a plus

Skills & Competencies:
Strong clinical assessment and diagnostic skills

Excellent communication and interpersonal skills

Ability to work independently and as part of a team

Compassionate, patient-focused approach to care

Proficiency in electronic health records (EHR) systems
