Social Worker
A skilled nursing facility is seeking a Social Worker to join their team.
Social Worker Job Description:
- Maintaining and/or improving the resident’s ability to control everyday physical needs and mental/psychosocial needs
- Identifying individual social and emotional needs
- Developing and maintaining individualized care plans
- Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
- Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
- Work with the finance team to ensure the resident is educated and the proper documents are in place for the resident during their stay
- Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community
Social Worker Qualifications/Requirements:
- BSW or MSW preferred, or Bachelors degree in the related field
- At least 3 years experience in the Social Work field required
- Ability to work flexible schedule
- PCC experience preferred
Salary: Up to $80,000 a year
