SALARY RATE: Negotiable (Annual Salary) based on the FTE

JOB DUTIES : The function of Rehab Services, is to provide services to residents/patients who have impairments, functional limitations, disabilities or changes in physical function and health status resulting from injury, disease or other causes, in compliance with Medicaid, Medicare, Omnibus Reconciliation Act (OBRA), Joint Commission (JC), Office of Healthcare Assurance (OHCA) and other regulatory and private agencies.

The primary purpose of this position is to coordinate, under the direction of the Regional Rehab Director, the overall needs, program, and quality of service provision of all the above stated areas of service. These include but are not limited too needs assessment, program development, quality assurance, budget development/monitoring, and personnel management.

Patient Care:

1. Provide physical therapy evaluation, treatment, prevention and education of disabilities, injuries and diseases. Treatment includes but is not limited to:

· Functional limitations that are preventing the patient from performing at their previous level of function before admittance to KCH.

· Education regarding their illness or disability, home exercise programs, equipment use, family/caregiver education.

· Safety and Prevention regarding functional mobility.

2. Provides thorough and complete Physical Therapy evaluations of patients/ residents of Kona Community Hospital and out-patients as prescribed by attending physician.

3. Demonstrates the ability to assess data, reflective of the patient’s status and interprets the appropriate information need to identify patient’s requirements relative to their age-specific needs

4. Develops a comprehensive rehabilitative Care Plan based on the patient’s evaluation.

5. Makes appropriate changes to the comprehensive rehabilitative care plan when needed with respect to clinical condition.

6. Coordinates with and makes appropriate referrals to other disciplines such as social services, occupational therapy, speech therapy and others as needed for the benefit of the patient.

7. Provides physical therapy treatment in accordance to the State of Hawaii Physical Therapy rules and regulations. Treatment will be consistent with the orders by the attending physician. All treatment plans will be indicated on the evaluation form and will fall within the guidelines of the rehabilitative care plan. The therapist may take verbal/phone physician orders.

8. Assists with Discharge Plans by communicating with the attending physician and others as appropriate when critical goals have been met and when the patient should be discharged from the hospital, or when physical therapy should be discontinued.

9. Sets up and instructs the patient/caregiver/family in an appropriate home program (family education.)

Supervisory Activities

Supervises PT IIIs, PTAs, Rehab Aides, and PT students to provide quality, progressive, and timely PT related services.

Plans, organizes, and directs the PT unit’s daily operations. Is the team leader of this unit and ensures that the services are timely, progressive, and provided with the highest level of quality.

THE MINIMUM QUALIFICATION REQUIREMENTS ARE: Applicants must meet all of the following requirements. Please note that unless specifically indicated; the required education and experiences may not be gained concurrently. In addition, qualifying work experiences are based on a 40-hour work week.

EDUCATION: Graduation from an accredited School of Physical Therapy. Doctoral Degree in Physical Therapy preferred

LICENSE REQUIRED: A valid unrevoked license with the State of Hawaii as a Physical Therapist at the time of appointment.

CERTIFICATIONS: Basic Life Support (BLS) for Healthcare Providers required at time of appointment

EXPERIENCE REQUIREMENT: Applicant must have progressively responsible work experience described below:

Minimum two (2) years working experience as a Physical Therapist under medical supervision. The experience must have equipped the applicant with a full professional understanding of the theories of prevention and treatment of physical disability or disease by physical therapy and their proper application by use of physical measures, activities and devices for preventive and therapeutic purposes which are appropriate to the patient’s physical and psychological condition. Supervisory experience preferred.

KNOWLEDGES AND ABILITIES REQUIRED:

· KNOWLEDGE OF: Anatomy, kinesiology, neuroanatomy, physiology, neurophysiology, and related sciences; principles and practices of physical therapy; physical therapy diagnostic evaluation and testing procedures; various treatment techniques; trends and developments in physical therapy; available physical therapy resources in the community; and, for some positions, principles and practices of supervision.

· ABILITY TO: Interpret and formulate operating policies and procedures, and plan, coordinate and conduct a physical therapy program; advise and counsel staff effectively; prepare a program budget; explain the program of services offered to clients and the public; maintain cooperative working relationships with clients, staff members and the community; and, for some positions, supervise the work of others.
