Position Summary
We are seeking a dedicated Hospice Registered Nurse (RN) Case Manager to provide skilled nursing care to hospice patients in accordance with physician orders, the State Nurse Practice Act, and agency policies.
This role involves case management, direct patient care, collaboration with an interdisciplinary team, and supporting patients and families through end-of-life care.
Key Responsibilities
- Provide direct nursing care to hospice patients in home and facility settings
- Manage patient caseload and oversee the full hospice care process
- Perform admissions, routine visits, evaluations, supervisory visits, and on-call duties
- Collaborate with the interdisciplinary team to develop and implement care plans
- Maintain accurate, timely, and compliant electronic documentation
- Educate patients and families on disease progression, care goals, and symptom management
- Participate in interdisciplinary team meetings and case conferences
- Conduct supervisory visits for aides and LPNs as assigned
- Support all levels of hospice care (routine, respite, continuous, inpatient, and imminent)
- Respond to changes in patient condition and ensure appropriate follow-up
- Participate in on-call rotation, including weekends and holidays
Qualifications
- Current Registered Nurse (RN) license in Oklahoma (required)
- Minimum of 2 years of hospice nursing experience preferred
- Hospice & Palliative Nursing Certification (preferred)
- Strong communication and documentation skills
- Ability to adapt in a fast-paced, ever-changing environment
- Valid driver’s license, reliable transportation, and active auto insurance
- Strong attention to detail and organizational skills
Work Environment
- Travel required to patient homes, assisted living facilities, and hospitals
- Flexible schedule with occasional irregular hours
- Collaborative, team-based environment
Physical Requirements
- Ability to lift up to 50 lbs and assist with patient mobility
- Ability to stand, walk, bend, and perform patient care tasks
- Adequate vision and hearing to assess patient needs and ensure safety
