Join Our Team:
We are seeking experienced and empathetic Hospice RN Case Managers to join our dedicated Scottsdale team. In this role, you will be essential in delivering high-quality hospice care and guiding patients and families through their journey with dignity and compassion.
Positions Available:
- 2 Full-Time RN Case Managers
- 1 covering the West Valley
- 1 covering the East Valley
Key Responsibilities:
- Manage and coordinate care for a caseload of hospice patients.
- Develop, implement, and update individualized care plans.
- Provide skilled nursing visits, including symptom management and end-of-life care.
- Educate patients, families, and caregivers on hospice philosophy and care expectations.
- Collaborate with an interdisciplinary team, including physicians, social workers, chaplains, and aides.
- Ensure accurate, timely documentation according to regulatory and company standards.
- Participate in on-call rotation as needed.
Qualifications:
- Current, unrestricted RN license in Arizona.
- Minimum of 1 year hospice experience preferred.
- Strong assessment, critical thinking, and communication skills.
- Compassionate, empathetic, and committed to high-quality end-of-life care.
- Reliable transportation, valid driver’s license, and insurance.
