Role Description
This is a full-time on-site position for a Hospice Case Manager/Registered Nurse (RN) based in Tempe, AZ. The Hospice Case Manager/RN will provide comprehensive nursing care to hospice patients, including assessing medical needs, developing care plans, coordinating with interdisciplinary teams, and implementing treatment plans. Responsibilities also include supporting patients and their families, managing symptom relief, and ensuring effective communication throughout the care journey. The role requires working closely with patients, families, and the hospice care team to deliver exceptional and compassionate care.
Qualifications
- Experience and expertise in Nursing and Medicine to assess, develop, and manage care plans.
- Proficiency in Case Management to coordinate care and provide holistic support to patients and families.
- Knowledge of Discharge Planning and Utilization Management to ensure smooth care transitions and optimized resource use.
- Strong communication and interpersonal skills to work effectively with patients, families, and multidisciplinary teams.
- Active and unrestricted Registered Nurse (RN) license in the state of Arizona.
- Experience in hospice, palliative care, or a related field is highly desirable.
- Ability to handle sensitive situations with compassion and professionalism.
