JAIL OUTREACH CASE MANAGER

SUMMARY.

The Jail Outreach Case Manager supports individuals transitioning from incarceration to Haven for Hope’s Transformational Campus and, ultimately, into stable housing in the community. The case manager is responsible for assessment, service coordination, and the delivery of recovery-oriented, therapeutic services to program participants (clients) while residing on Campus. The role requires focused attention to the complex needs of justice-involved individuals, as well as those experiencing chronic homelessness, mental and physical health challenges, substance use disorders, and other traumas.

In collaboration with the Jail Outreach team, the Case Manager helps participants meet community supervision requirements while engaging in programming that supports long-term stability. The case manager delivers targeted, time-limited interventions aimed at building life skills and connecting clients to community supports that promote stability, recovery, and housing retention. Success in this role requires creativity, strong collaboration, excellent organizational skills, and a commitment to person-centered care.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES.

  • Must regularly visit Bexar County jail or conduct virtual visits to make initial contact with referrals and complete assessments.
  • Completes Intake processes related to enrollment, to include verifying program and campus eligibility, scheduling enrollment and bed assignment, completing HMIS enrollment workflows and uploading necessary documentation pertaining to Intake and Jail Outreach program requirements.
  • Conducts ongoing assessments, collaboratively with clients and team, to guide ethical and targeted service delivery in the formulation of comprehensive, person-centered service plans that are reviewed and modified with the client and team on a regular basis.
  • Provides ongoing case management and individually tailored services in conjunction with other professionals involved in client care and programming, including assistance with referrals to appropriate supports that promote each client’s recovery, wellness, and housing goals. Tailored services may include housing placement, independent living skills, vocational guidance, appointment escorts, and integrated substance abuse and mental/physical health treatment.
  • Works collaboratively, to include involvement in staffing, Recovery Team Meetings, and ongoing communication about client strengths and needs.

 

  • Willing to use shared, lived experience with those served through the program as a way to offer hope and direction in meeting their goals.
  • Develops and manages a comprehensive outreach/working relationship with stakeholders, including on-campus and community-based partners, while maintaining thorough knowledge of area resources for referrals such as financial, mental health, substance abuse, and alternative housing services.
  • Assists clients with managing their monies, including preparing budgets with clients and computing expenses.
  • Educates, informs, and advocates for clients regarding benefits and entitlements.
  • Completes fiscal processes and check requests to support clients’ comprehensive needs.
  • Maintains compliance with written and computerized records, compiles reports and completes other program documentation, to include case notes, data management, letters, grant requirements.
  • Participates in training opportunities to develop skills utilizing Motivational Interviewing, Trauma Informed Care, and Recovery Oriented Care.
  • Support the goals of the Haven for Hope Campus, promoting a positive Campus image to the local community and adhering to the agency mission as well as demonstration of following a professional code of ethics.
  • Other duties as assigned.
Qualifications:

EDUCATION.

  • Minimum High School Diploma or GED.
  • Preferred Bachelor’s degree in a Human/Social Services, Psychology, Social Work, or related field.

 

EXPERIENCE.

Strong candidates will have experience working with justice involved, former foster youth, aging adults, mental and physical health challenges, and those experiencing chronic homeless. Three or more years’ experience in previous roles of direct case management or outreach and engagement and shared, lived experience is preferred. Knowledge and application of case management skills essential with the ability to balance and prioritize simultaneous tasks in a fast-paced and changing environment.

  • Ability to review, learn, follow, and adapt evidenced based treatment programs to deliver to diverse client groups.

  • Excellent communication skills, written and verbal, to represent Haven for Hope and support clients with histories of homelessness and traumas.
  • Excellent administrative ability, including organizational skills, attention to detail and strong computer skills. Ability to navigate in Microsoft Office and to learn and maintain information systems developed specifically for Haven for Hope.  Ability to learn and apply concurrent documentation techniques is essential. 
  • Ability to be self-directed and to take initiative, which includes ability to discern when to seek supervision
  • Strong creative and critical thinking skills, to include the ability to analyze problems, identify alternative solutions, and project consequences of proposed actions.
  • Ability to work collaboratively and be invested as a member of the assigned H4H Team.
  • Knowledge of local housing programs and requirements, and homeless services within the Bexar County area
  • Familiarity with Federal and State Local Agencies involved in housing, and housing policy.
  • Shared Lived Experience of homelessness, mental health and/or substance abuse challenges is an additional asset to the position.

 

CERTIFICATES, LICENSES, REGISTRATIONS

Prefer a valid Driver’s License, a good driving record, reliable transportation and proper insurance coverage on automobile used for work. Monthly reimbursement for applicable and documented work related expenditures is provided.  Travel within the city and its surrounding areas will be required (if applicable).

Standard physical activity such as periodic standing, walking, sitting, basic motor skills to access computer files for information or simple data entry. Occasional vigorous physical activity such as carrying supplies, loading supplies in the van, working in extreme weather, having to lift more than 50lbs can be expected.
