What You'll Do

As a Home Health Physical Therapist, you will:

• Evaluate patients and develop individualized treatment plans
• Provide skilled physical therapy services in patients' homes
• Collaborate with physicians, nurses, therapists, and other interdisciplinary team members
• Educate patients and caregivers to promote safe recovery and independence
• Monitor progress and adjust treatment plans as needed
• Supervise and collaborate with Physical Therapy Assistants (PTAs)
• Maintain timely and accurate clinical documentation
• Help patients achieve their highest level of function and quality of life

Qualifications

Required:

• Bachelor's Degree in Physical Therapy from an accredited program
• Current Connecticut Physical Therapist License
• Minimum 1 year of clinical experience
• Valid driver's license and reliable transportation

Preferred:

• 3 years of Physical Therapy experience
• Home health experience
