What You'll Do
As a Home Health Physical Therapist, you will:
- Evaluate and treat homebound patients referred for physical therapy services
- Develop individualized treatment plans designed to maximize patient independence and function
- Assess strength, balance, mobility, endurance, gait, pain, and activities of daily living
- Deliver evidence-based therapeutic interventions and exercises
- Collaborate with physicians, nurses, and interdisciplinary care teams
- Educate patients and caregivers to support recovery and long-term success
- Monitor progress and adjust treatment plans as needed

Qualifications
Required
- Graduate of an accredited Physical Therapy program (Bachelor's or Entry-Level Master's Degree)
- Active Connecticut Physical Therapist License
- CPR Certification (required by start date)
- Prior Physical Therapy experience
