What are the responsibilities and job description for the Social Worker position at Harrison County Health Department & Hospice?
Hospice Social Worker-required degree in Masters or Bachelors of social work, psychology, sociology, family counseling
The ideal candidate will be compassionate, have a high level of interpersonal skills and have previous hospice or medical field experience. The ability to function as a member of an interdisciplinary team as well as the skill to work autonomously is essential. The Social Worker provides direct patient care and is responsible for completing psychosocial assessment and develops a plan of care. Common work activities are assisting with completion of Healthcare Directives or other paperwork/applications, identifying end-of-life tasks, identifying and accessing community resources, providing education, problem-solving, emotional support and intervention as relates to dealing with loss of independence, coping with anticipatory grief and bereavement work. The Social Worker will provide bereavement services and facilitate a Grief Support Group. The Social Worker will also coordinate Hospice Volunteers.
Generally, the Social Worker and hospice team assists the patient/caregiver in establishing a safe environment, supports independence and assists in both defining and achieving the patient's quality of life.
This position requires flexibility and the ability to work in what is often a fast-paced, demanding environment. The ability to prioritize needs/tasks, critical thinking skills, and emotional maturity are essential to success in this position. Crisis interventions skills are a plus.
As Harrison County Hospice covers a large portion of Northwest Missouri, it is necessary to have reliable transportation.
While a master's level SW degree is preferred, a bachelor's level degree with a background in hospice or health care will be considered.
The physical demands and the work environment are representative of those that must be met by an employee to successfully perform the essential functions of a social worker in the home, office, or clinic. The employee is occasionally exposed to outside weather conditions. The employee must occasionally lift and/or move up to 50 pounds. The employee must be able to drive and hold a valid driver's license. Automobile insurance is the employee's responsibility. Compensation is given for approved miles driven.
