Harmonie House is a nonprofit organization dedicated to supporting children and adults with disabilities across Northwest Colorado and surrounding regions. We help individuals and families navigate Medicaid and Waiver programs, build independence, and access services that improve quality of life.

Position Summary
We are seeking a Care Coordinator to support clients and families in accessing services, navigating systems, and maintaining high-quality, compliant care. This role includes direct client interaction, care planning, coordination with internal and external partners, and ongoing monitoring of services.

This role requires a high level of independence, initiative, and accountability, while working within a supportive team environment that provides ongoing guidance, collaboration, and oversight. The ideal candidate is compassionate, highly organized, and able to manage multiple responsibilities in a fast-paced, mission-driven environment.

Key Responsibilities

  • Manage an assigned caseload, serving as the primary point of contact and coordinating all communication with clients, families, caregivers, and partner organizations
  • Maintain regular, proactive communication to ensure client needs are addressed and services remain aligned with care plans
  • Assist clients and families with Medicaid and waiver applications and system navigation
  • Conduct intake assessments and participate in care planning and service plan development
  • Enroll clients into Harmonie House services and maintain accurate, audit-ready documentation
  • Conduct home visits and community-based visits across multiple counties
  • Monitor service delivery through regular check-ins, documentation review, and quality oversight
  • Maintain caregiver schedules and track service utilization
  • Coordinate with Case Management Agencies, healthcare providers, and community partners
  • Ensure services are delivered in alignment with care plans, authorizations, and regulatory requirements
  • Monitor compliance with HCPF, Medicaid, and program requirements
  • Participate in interdisciplinary team meetings and case coordination meetings
  • Support communication and coordination across caregivers, families, and partner organizations
  • Identify and connect clients to additional community resources and supports
  • Maintain timely, accurate documentation including case notes, assessments, and correspondence

Qualifications

  • Associate’s degree in Human Services, Healthcare, or a related field, or a minimum of four (4) years of relevant experience in human services, healthcare, or a related field required
  • Bachelor’s degree in Human Services, Healthcare, or a related field preferred
  • Experience working with children and/or adults with developmental disabilities required
  • Experience in healthcare, case management, social services, or a related field strongly preferred
  • Familiarity with Medicaid, waiver services, or disability service systems is highly desirable
  • Strong organizational, communication, and problem-solving skills
  • Ability to manage multiple priorities and work both independently and as part of a team
  • Comfortable with technology, documentation systems, and communication tools
  • Valid driver’s license and reliable transportation required
