- Administer medications and treatments as prescribed
- Monitor residents’ health status and report changes to the care team
- Collaborate with CNAs, RNs, and interdisciplinary team members
- Ensure a high standard of care focused on dignity, comfort, and well-being
- Maintain accurate and timely documentation
- Current LPN license in the state of Kentucky
- Strong clinical skills and attention to detail
- Excellent communication and interpersonal abilities
- A genuine passion for caring for others
- Long-term care experience preferred, but not required
- You prioritize resident-centered care
