The Opportunity

We are currently seeking a dedicated LPN VA Liaison, Charge Nurse to join our team! As a VA Liaison, you will be responsible for providing exceptional care to our veteran residents, ensuring their needs are met, and coordinating with the Veterans Affairs department in Atlanta.

Responsibilities

• Veteran Resident Authorization process and follow ups

• Assure VA resident needs are followed up, including any VA grievances, family grievances

• Coordinate VA communication for any VA resident appointments, transportation needs, and needs while in the facility

• Support activities of daily living

• Maintain clear, timely documentation

• Communicate effectively with the interdisciplinary teams and families

• Adhere to infection control and safety standards to protect your residents and your teammates

Requirements

• Active LPN license in good standing in the state of practice

• Experience in a skilled nursing or long-term care setting is preferred but not required
