The LPN Hospice Liaison serves as the primary clinical representative for Hospice services within hospitals, healthcare facilities, and physician practices. This role focuses on identifying patients who may benefit from hospice care, educating patients and families about hospice services, facilitating timely admissions, and collaborating with healthcare providers to ensure a smooth transition from curative treatment to confirm-focused care. The Hospice Nurse Liaison acts as a clinical resource and advocate while promoting hospice services in accordance with regulatory and organizational standards.
