What are the responsibilities and job description for the Coordinator - Supportive Services Case Manager position at Harbor Brand?
Harbor is seeking a competent professional to coordinate supportive services and CPST/TBS services to transition age youth (18-24 yrs old) in a permanent supportive housing environment who are at risk of homelessness. This position supports youth in developing skills for living independently, managing a household, and setting individualized educational and career goals.
Position is full-time, 40 hours per week, and works from 10a - 8p.
Education/Experience/Other Requirements:
- High School Diploma and 3 years of experience required. Bachelor’s degree in Counseling, Social Work, Psychology, Nursing or closely related behavioral/mental health field preferred
- Must be proficient in the use of technology, adept at Windows and Microsoft based programs and able to generate and interpret performance data.
- Must successfully complete required training, before services are rendered, in basic
competencies including, at a minimum, an understanding of mental illness or
substance use disorder treatment and recovery, psychiatric and substance use
disorder symptoms and their impact on functioning and behavior; how to
therapeutically engage a person with mental illness or a person in substance use
disorder treatment and recovery; crisis response procedures; an understanding of the
community behavioral health system, social service systems, criminal justice system
and other healthcare systems; de-escalation techniques and understanding how your
own behavior can impact the behavior of others - Must have a commitment to delivering excellent customer service.
- Able to effectively supervise, provide participant focused services and motivate staff to achieve maximum results.
- The skills required include strong and effective oral and written communication skills, organizational skills, and time management skills.
- Must be willing to work nights and weekends as needed.
- When applicable, non-licensed staff will be required to complete 20 hours of CEUs every 24 months.
- CPR/First Aid and NCI/CPI Certifications required within 90 days of employment
Essential Job Competencies/Primary Duties:

- Provides direct services and supports to residents in developing and individualized service plan and achieving goals. and wellbeing.
- Supports residents with appropriate referrals to agencies/programs in support of service plan.
- Provides education and direct services to residents to promote independent living skills, achieving educational goals, exploring careers, and gaining employment.
- Supports residents in problem-solving and resolving tenant issues to successfully maintain housing.
- Follows permanent supportive housing model in providing support services to residents.
- Works collaboratively with building management team to support program goals.
- Follows emergency procedures and incident reporting to insure resident safety.
- Supports resident meetings for input and supports a continual improvement process for delivering permanent supportive housing services.
- Attends meetings with building management team to support coordinated service delivery.
- Assists residents with meetings with building management team and supports residents in understanding lease, resident building expectations, to insure residents follow building rules.
- Attends vocational staff, general staff, and other meetings as appropriate.
- Keeps current with trends and developments related to essential job competencies, and demonstrates continued growth.
- Must be willing to work flexible hours which may include evenings and/or weekends.
- May be required to work at multiple sites, or provide services in the home or other community setting.
- Meets billing expectations and follows agency requirements for documentation and service reporting.
