Job Overview:
Hanford Home Health is seeking a dedicated and compassionate Full Time Home Health Skilled Nurse (RN) to provide in-home, skilled nursing care to EEOICPA (Department of Labor) benefits recipients and other medically complex patients. As a key member of our team, you will have the opportunity to make a meaningful impact on the lives of our patients and their families. This position reports to the Director of Clinical Services.
Schedule & Shift Structure:
Full-time status is 36-40 hours per week
Shifts range from 1 to 12 hours, based on patient needs and census
Full-time staff are primarily scheduled for 12-hour shifts
Alternative shift lengths may occur only by mutual agreement
Day Shift: 7:00 AM – 7:00 PM
Night Shift: 7:00 PM – 7:00 AM
Staffing Model:
Care is primarily one-to-one
In limited circumstances, staff may support more than one patient within the same household when authorized by the individualized Plan of Care
Work schedules are posted one (1) month or more in advance
Staff are expected to complete all assigned shifts accepted, subject to agency attendance and call-out policies
Weekend & Holiday Expectations:
Full-time staff may be required to work weekends and holidays in accordance with agency policy, operational needs, and census
Weekend and holiday assignments are scheduled and rotated per agency policy
Schedules are subject to census changes as outlined in the Employment Contract
Key Responsibilities:
Provide skilled nursing care in accordance with the authorizing practitioner's orders and the individualized Plan of Care
Perform nursing assessments and ongoing monitoring within RN scope of practice
Administer medications and treatments; monitor for side effects and changes in patient condition
Provide skilled interventions as ordered (including wound care and other therapeutic interventions when within scope and ordered)
Measure and document vital signs, intake/output as applicable, pain, weight, and patient responses per the Plan of Care
Observe and report changes in patient condition to the supervising nurse, RN Case Manager, and/or Director of Clinical Services as required
Educate patients and families on conditions, medications, treatments, safety measures, and Plan of Care requirements
Collaborate with interdisciplinary team members to coordinate care and support optimal patient outcomes
Assist with mobility, personal care, ADLs, and IADLs when clinically indicated and outlined in the Plan of Care
Complete accurate, timely documentation in the agency-approved electronic health record system
Follow infection control, safety, emergency preparedness, confidentiality, and HIPAA requirements
Required Qualifications:
Active Washington State RN license in good standing
Graduate of an accredited School of Nursing
Minimum of one (1) year of nursing experience; home health experience preferred
Current CPR/BLS certification for healthcare professionals
Washington State Food Worker's Card (or ability to obtain upon hire)
Valid driver's license, reliable transportation, and auto insurance
Ability to pass background check and drug screening
Strong clinical judgment, documentation, and communication skills
Preferred Qualifications:
Experience in home health, hospice, or geriatric care
Prior experience with electronic documentation systems
Pay & Benefits:
Pay Rate: Base rate $55.00 per hour
Actual wage may vary based on assignment, credentials, experience, and applicable shift differentials
Full-time skilled nurses are eligible for annual wage increases in accordance with agency policies and procedures
This position is nonexempt (hourly). Overtime is paid in accordance with Washington State and federal law. Overtime pay and applicable shift differentials are paid in addition to the base hourly rate
Shift differential pay per agency policy
Mandatory Washington meal and rest periods are paid and based on Washington State required break and meal periods
Medical, dental, and vision insurance for employees working 36 hours per week
401(k) eligibility after 6 months of service for employees working 20 hours per week
Travel required within the Tri Cities and surrounding areas; mileage reimbursed at $0.725 per mile
Washington Paid Sick Leave provided in accordance with Washington State law
One (1) set of agency-issued scrubs for full-time staff
Reimbursement for required CPR/BLS certification per agency policy
Opportunity for additional shifts as available
