Pay range:$23 -$27 per hour
Hands at Home Care Services is seeking reliable, compassionate Per Diem Caregivers to support clients in their homes. This role is perfect for someone who wants true flexibility while still being dependable, consistent, and committed to excellent care.
Per Diem caregivers fill gap shifts, same-day/next-day coverage, short shifts, and temporary coverage. This is not a guaranteed-hours position and is not for someone seeking recurring weekly schedules.
What Makes This Per Diem Role Different
To protect clients and our core caregiving team, Per Diem caregivers must:
- Be consistently reliable once they accept a shift
- Complete all documentation during or immediately after each visit
- Feel comfortable with urgent or short-notice coverage
- Adapt quickly to different client homes and care needs
- Work at least 20 hours per month to remain active
- Communicate clearly and promptly with the office
This ensures per diem flexibility never compromises care quality.
Responsibilities:
- Provide personal care and ADL support
- Assist with mobility, transfers, and fall prevention
- Support clients with meals, hydration, and light household tasks
- Follow individualized care plans
- Document all ADLs and care notes in AxisCare before the shift closes
- Communicate any concerns or changes promptly
- Provide temporary or short-notice backup coverage
- Maintain professional boundaries and confidentiality
Requirements:
- Previous caregiving or facility experience preferred (Caregiver, LNA, PSS, DSP, PCA, CNA, etc.)
- Ability to work some evenings, weekends, or short shifts
- Reliable transportation
- Strong communication and documentation skills
- Ability to work independently in client homes
- Comfort working with a variety of client personalities and needs
- Dependable attendance once shifts are accepted
Schedule Expectations This Per Diem role offers flexibility:
- You choose when you are available
- You pick up open shifts through our scheduling team
- You must work 20 hours per month minimum
- You will not receive a set weekly schedule
- You will not be assigned recurring weekly clients
Common assignments include:
- Same-day shifts
- Next-day coverage
- Short shifts (1–4 hours)
- Vacation/leave coverage
High-acuity clients are assigned only when appropriate skills and competency are verified.
Training & Onboarding We provide a streamlined onboarding track:
- Quick orientation to policies and documentation
- Simple ADL and care-note training
- Skill verification before client assignments
- Limited shadow shifts (only if required for safety)
