Location: Client’s Home- Campbell County, LaFollette Tennessee (TN)
Status: PRN, Full-Time or Part-Time | Contract 1099

Position Summary:
We are seeking a dedicated Licensed Practical Nurse (LPN) to provide skilled nursing services, monitor client health, and ensure high-quality care according to individualized plans. This role collaborates with RN Case Managers, caregivers, and interdisciplinary teams to promote health, safety, and independence.
Key Responsibilities:
- Perform skilled nursing assessments and implement care plans under RN direction.
- Administer medications and treatments safely and accurately.
- Complete clinical documentation and report changes in client condition promptly.
- Communicate effectively with clients, families, and care teams to ensure continuity of care.
Qualifications:
- Graduate of an accredited LPN program.
- Active LPN license in good standing in Tennessee.
- Minimum 1 year of nursing experience preferred; home health experience a plus.
- Strong clinical judgment, documentation skills, and ability to work independently.
