Location: Client’s Home- Oak Ridge, Tennessee (TN)
Status: PRN, Full-Time or Part-Time | Contract 1099

Position Summary:
We are seeking a dedicated Licensed Practical Nurse (LPN) to provide skilled nursing services, monitor client health, and ensure high-quality care according to individualized plans. This role collaborates with RN Case Managers, caregivers, and interdisciplinary teams to promote health, safety, and independence.
Key Responsibilities:

Perform skilled nursing assessments and implement care plans under RN direction.

Administer medications and treatments safely and accurately.

Complete clinical documentation and report changes in client condition promptly.

Communicate effectively with clients, families, and care teams to ensure continuity of care.
Qualifications:

Graduate of an accredited LPN program.

Active LPN license in good standing in Tennessee.

Minimum 1 year of nursing experience preferred; home health experience a plus.

Strong clinical judgment, documentation skills, and ability to work independently.
