What are the responsibilities and job description for the Community Home Health LPN position at Hancock County Health System?
RESPONSIBILITIES:
Provides skilled nursing care to patients in their homes under the supervision of a Registered Nurse. This involves direct patient care, education, and coordination of services to support patients' health and independence. Works as a resource to communities in Hancock County to provide assistance where needed for Public Health. Will travel to patient homes as needed.
HOURS:
Full-time, Monday-Friday, shift 8:00am-4:00pm.
QUALIFICATIONS:
Graduate from an accredited School of Nursing and must be currently licensed as an LPN in the State of Iowa. One year's experience in a health care setting required. Must be able to work independently in a variety of home situations. Must have valid driver's license and own vehicle.
PHYSICAL REQUIREMENTS:
Must be able to stand for extended periods of time. Must be able to have good visual and auditory skills.
