Role Description

This is a full-time on-site role for a Case Manager located in Queens, NY. The Case Manager will provide direct support to clients by conducting assessments, developing care plans, and ensuring access to appropriate services and resources. Responsibilities include creating and maintaining client records, collaborating with service providers, advocating on behalf of clients, and monitoring progress to ensure successful outcomes. The Case Manager will also assist in community outreach initiatives and participate in program development efforts to address client needs effectively.


Qualifications

  • Experience in client assessments, case management, and service coordination
  • Strong organizational and time management skills to handle case documentation and reporting
  • Excellent interpersonal, communication, and advocacy skills to build relationships with clients and community resources
  • Knowledge of social services systems, including services for youth, seniors, immigrants, and housing programs
  • Proficiency in basic computer applications, such as Microsoft Office Suite
  • Bilingual skills (English and another language, such as Greek or Spanish) are a plus
  • Minimum of a Bachelor’s degree in Social Work, Human Services, Psychology, or a related field
  • Familiarity with the Queens, NY community and its resources is preferred
