Case Manager, Hampton Behavioral Health Docket

Annual Salary: $50,604

Work Schedule: Monday - Friday, 8:30 am - 5:00 pm

Become part of the Hampton-Newport News Community Services Board as a full-time Case Manager supporting the Behavioral Health Docket (BHD) program. In this onsite position, you'll collaborate with an experienced multidisciplinary team that includes licensed clinicians and behavioral health professionals to coordinate services for individuals with behavioral health needs. Our Organization

Since 1971, the Hampton-Newport News Community Services Board has provided a comprehensive continuum of high-quality, essential, and community-based services to citizens of the Peninsula who are living with a mental illness, developmental disabilities, and substance use disorders.

HNNCSB partners with both Hampton and Newport News General District Courts as part of separate dockets. BHDs are non-adversarial approaches by the judicial system, treatment providers, and other stakeholders addressing the needs of individuals with serious mental illness who have become involved in the criminal justice system. Through on-going support and supervision, the Court and Team assist individuals as they engage in services and address challenges to prevent further penetration within the criminal justice system.

Your day as a Case Manager for the Behavioral Health Docket

As a Case Manager with the Hampton Behavioral Health Docket (BHD), you will help adults with Serious Mental Illness (SMI) and Serious and Persistent Mental Illness (SPMI) access the services and support they need to live successfully in the community.

Responsibilities

  • Assess client needs and coordinate services.
  • Develop and monitor individualized service plans.
  • Connect clients with community resources.
  • Provide advocacy, crisis intervention, and supportive case management.
  • Collaborate with treatment providers, families, and criminal justice partners.
  • Maintain accurate documentation and client records.

Make a meaningful impact by helping individuals achieve stability, recovery, and independence.

Requirements

  • Bachelor's degree in human services such as Social Work, Psychology, Sociology, or Counseling.
  • One (1) year experience in case management services.
  • Valid Virginia Driver's License.
