About the Assertive Community Treatment (ACT) Program

Assertive Community Treatment (ACT) is a multidisciplinary team of medical, behavioral health, and rehabilitative professionals who work collaboratively to meet the complex needs of individuals with severe and persistent mental illness-predominantly Schizophrenia-and high service needs to maintain stability in the community.

ACT provides a wide range of services, including service coordination, psychiatric assessment and treatment, medication management, substance use services, counseling and therapy, vocational services, mental health support services, peer support, and representative payee services. The program's flexible model ensures that individuals receive the right level of care based on their changing needs over time.

The primary goal of ACT is to lessen or eliminate the debilitating symptoms of mental illness, minimize recurrent episodes, and help individuals meet their basic needs, improve social and vocational functioning, increase stability in the community, and reduce hospitalizations-ultimately enhancing quality of life for both clients and their families.
