Homecare Specialist

Job Overview:

The Homecare Specialist is responsible for the complete coordination of home care benefits which include PCA, CDPAS services from the initial referral for services. Ensuring members receive qualifying service under the plan. Secure coverage with our in-network providers, request authorizations and follow up on start of care for services secured. Responsibilities include coordination of homecare services for new members, PPL referrals for new and existing CDPAS members. Ongoing communication with all homecare providers regarding services that are already in place for members. 

Essential Responsibilities:

• Securing PCA coverage for all members within assigned counties, aiming to have no members on the open case list.
• Accepting/addressing activities from the Care Managers (Members in need of coverage, retention, schedule changes, hospital discharges, members new to CPAS, etc.)
• Refer new and existing CDPAS enrollees to PPL. 

• Call vendors to follow up on staffing requests. 

• Document vendor communications in member's chart.

• Notify authorization department/send authorization requests to authorization team.

• Other duties as identified as a necessary part of employee’s role

Specific Knowledge, Skills, and Abilities:

• Proficient in the use of office equipment and the computer; Microsoft Word and Excel.

• Good organizational and interpersonal skills.

• Demonstrates good customer skills on telephone and email.
