The Supportive Housing Case Manager provides a single point of accountability for coordination of services to homeless individuals and families, including survivors of domestic violence. Housing Case Managers help tenants achieve their goals and meet their needs through the provision of, or linkage to, a variety of services. Housing Case Managers provide comprehensive services that are tailored to meet the biological, psychological and social needs of each tenant to help tenants remain stably housed in apartments in a scattered site model.
Knowledge of Motivational Interviewing and risk assessment. Works with tenants to develop and monitor Housing Stabilization Plans (HSP) which include educational, vocational, medical, life skills and housing goals. Assist program participants to obtain resources and services as requested and/or needed to address barriers to housing as stated above including but not limited to CORI mitigation, credit repair, and assistance with financial applications.
ESSENTIAL FUNCTIONS:
· Use Motivational Interviewing techniques to develop Housing Stabilization Plans (HSP) which include financial, mental health, physical health, legal, educational, vocational, and life skills goals.
· Meet with tenants regularly (no less than monthly) to review HSP progress and plan next steps. Many of the clients have a disability and require knowledge of medications and medical issues.
· Make referrals to community programs and assist tenants in obtaining resources and services for areas on the tenant’s HSP and as needed to address barriers to independent living.
· Connect tenants with educational and vocational resources to help improve employment opportunities.
· Develop partnerships and serve as a liaison with community-based organizations and attend meetings with such organizations as needed.
· May assist tenants in finding housing options.
· Assist tenants with move-in transition supports, including but not limited to: furniture and furnishings assistance, budget support, and other resources and services as needed.
· Document all interactions with tenants utilizing appropriate reporting mechanisms, including our Housing Management Information System (HMIS).
· Attend staff meetings, clinical supervision, and training as required.
· Attend community meetings at least once per month.
· Complete reports for program as needed.
· Perform other tasks as requested by the Program Manager, or the Executive Director.
· Observe all agency rules and policies.

EDUCATION, KNOWLEDGE, SKILLS, ABILITIES, AND EXPERIENCE
· Communicate effectively, orally and in writing.

REQUIRED/PREFERRED:
· Associate’s degree and four years related work experience or bachelor’s degree with two years related work experience (required). Master’s degree preferred.
· Experience with crisis intervention regarding issues such as homelessness, mental health, and substance abuse; housing barriers including legal, financial, psychological and criminal (preferred).
· Knowledge and experience with service agencies and resources for homeless individuals and/or families (preferred).
· Willingness to accompany tenants as needed to appointments (required).
· Willingness to work as part of a team to promote the goals of the agency (required).
· Sensitive to the needs of the homeless, low income and diverse populations (required).
· Ability to work in a busy, diverse team setting (required)
· Excellent time management, organizational and communication skills (required).
· Proficient in the use and operation of computers and basic Microsoft applications (Word, Excel, Access, Outlook) (required).
· Executive Director can override requirements above.
LICENSES AND CERTIFICATES:
Must possess a valid Florida driver’s license/minimum vehicle insurance required by law and maintained throughout the duration of employment.
PHYSICAL REQUIREMENTS:
· The working environment for this position is an office that is mostly clean and comfortable. It may include some minor annoyances such as noise, odors, drafts, etc. The incumbent is in a non-confined office-type setting in which he or she is free to move about at will.
· While performing this job, the incumbent typically spends time standing, lift up to 25 pounds walking, sitting, listening/speaking, reading and writing.
· Home visits using personal transportation are required.
HIRING REQUIREMENTS:
1. Must be able to pass a level II background and drug test clearance.
2. Must possess a valid Florida driver’s license/minimum vehicle insurance, and a reliable vehicle required by law and maintained throughout the duration of employment. Please note: Upon job offering a current Florida driver’s license and a driving record acceptable to the Agency's insurance carrier is required.
3. Own reliable transportation is required.
