POSITION SUMMARY

This position is responsible for the delivery of outreach services and activities to the target population. The position is expected to work within the program and policy directives of the Wai‘anae Neighborhood Place of the Wai‘anae Coast Community Mental Health Center, Inc. / Hale Na‘au Pono.

REPORTS TO

Administrative: Program Director, Wai‘anae Neighborhood Place Director. This position is guided by the job description, Agency and program policies procedures, applicable State and Federal laws, contract requirements, and the standards and requirements of the pertinent professional organizations and disciplines.

CLASSIFICATION

Part- or Full-Time. Non-Exempt

COMPENSATION

Hourly:  $16.50 - $20.00

Program Mission

To provide individuals and families outreach services that build capacity, reduce environmental and social risk factors while increasing protective factors and access to resources.

Target Population

Families and individuals needing concrete and informational support, improvement in health and well-being.

 

I. CORE FUNCTIONS

Administrative Duties

  1. Welcomes visitors and callers to identify needs
  2. Takes messages and forwards to staff
  3. Retrieves and sends mail from the main office for processing
  4. Documents encounters and interactions
  5. Prepares and maintain client folders and action logs
  6. Support in the planning and development of outreach events during working hours, after hours, and on weekends, as required.
  7. Maintains privacy and personally identifiable information (PII) security standards of the persons served and an ethical code of conduct at all times.
  8. Receives assignments for the week and/or by the day, from the Program Director.
  9. Open and closing offices and general maintenance of WNP work areas, including conference rooms and Donation Closet.
  10. Complete mandatory and assigned training
  11. Perform general clerical duties such as word processing, correspondence, e-mail, telephone and in-person reception, filing, data entry, record maintenance, faxing, scanning and timely and accurate data entry.

Program Duties

  1. Provides both direct and outreach services to the persons served as a part of Wai‘anae Neighborhood Place. 
  2. Respects the cultural values and beliefs of persons served. 
  3. Assists with the intake/application process with the persons served in completing demographic and authorization, consents and other necessary information.
  4. Conduct outreach activities to reach the target population, including, but not limited to, canvassing the community, distribution educational material, making public presentations, and staffing fair booths.
  5. Completes weekly, monthly and annual reporting responsibilities.
  6. Provides referrals to any applicable resources, or any other appropriate State agency or assistance programs, and maintains a list of, and establish a protocol for, referral services available to assist the persons served with questions, grievances, complaints or questions about assistance and/or services.
  7. Educates, assists, and refer the persons served with other educational, legal, social, emotional, medical, behavioral, vocational, entitlements, and/or other human services support needs to the appropriate advocacy, education, or service-provider organizations.
  8. Supports the program’s quality activities as well as the Agency’s Performance and Quality Improvement Plan.
  1. Operates, as needed, the Donation Closet.
  2. Provides both direct and outreach with the persons served, as needed, when the assigned worker is not available.

Family Outreach

  1. Provides face to face, collateral contact, documents applicable information and submitting progress notes within one working day.
  2. Coordinates activities with other services providers, as appropriate, to accomplish joint family service planning and community outreach events.
  3. Maintains a family caseload of 20, which involves maintaining case records of persons served according to policy and making a face-to-face contact according to the service plan.
  4. Administer the Family Assessment Tool with enrolled families.
  5. Develop and implement a service plan with enrolled families.
  6. Provides outreach, information and referral assistance with enrolled families, visitors and callers to Neighborhood Place.  Family Outreach services include assisting persons with multiple issues including financial, educational, food security, domestic violence, abuse, disability, eviction, health needs.



II. OTHER FUNCTIONS

  1. Behave in a manner consistent with Hale Na‘au Pono’s mission, job description, agency and program policies & procedures, applicable state and federal laws, contract requirements, and the standards and requirements of the pertinent professional organizations and disciplines.
  2. Performs other related duties as assigned.

III. QUALIFICATIONS

Knowledge / Abilities

  1. Possess empathy, cultural competence and understand the needs of households on the Waiʻanae Coast.
  2. Ability to communicate effectively with community members public/private sector officials, professional, para-professional, administrative support, participants and family members.
  3. Demonstrate ability to collaborate effectively in a team setting
  4. Demonstrate a high level of creativity and flexibility to promote, enhance and attain program goals.
  5. Demonstrate knowledge and effective utilization of community resources.
  6. Proficient in using the internet, computers, Microsoft Office software, internet hotspots, projectors and basic office equipment.
  7. Ability to work independently in a fast-paced and ever changing environment

Education & Experience

  1. High school diploma/GED

 

  1. And four (4) years of experience working with the target population, in a human, social, health or other related field.
  2. And an approved waiver from DHS if applicant educational credential is a high school diploma/GED.

Preferred Qualifications

  1. Bachelor’s degree in social work, psychology or related field required.
  2. And two (2) years of experience working directly with the targeted population.
  3. Experience providing outreach and doing in-person and virtual presentations.

Physical Requirements

  1. Must be physically able to perform job duties with or without reasonable accommodation.  Any physical condition that could cause the applicant to be a hazard to themselves or others, will disqualify them for appointment.

IV. OTHER REQUIREMENTS

  1. Valid driver's license and clean traffic abstract within the past three years and ready access to an insured vehicle.
  2. Availability to work flexible hours, including nights, weekends and holidays.
  3. Provide consent and necessary information for a criminal background checks on an initial and bi-annual basis.
  4. Consents and submits to drug testing.

Work Environment and Physical Demands:
 This job operates in a professional office environment. The role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines. The employee is occasionally required to stand; walk; reach with hands and arms; and stoop, kneel, or crouch.

 

Work Hours and Travel Requirements:
 This position requires availability to work flexible hours, including nights, weekends, and holidays. Travel is primarily local during the business day, although some out-of-area and overnight travel may be expected.

 

Professional Development and Training:
 The Position is expected to participate in ongoing professional development opportunities and attend relevant workshops and training sessions as deemed necessary by the Supervisor.
