Role Overview
An Outreach Case Worker is to provide comprehensive, client-centered, goal-oriented process for assessing the need of a young person for particular services, assisting him/her/them to obtain and complete those services and, at a systems level, to coordinate the provisions of services with community resources available to the young person. The individual in this position will conduct business in a professional and cordial manner that will uphold the integrity and reputation of Hale Kipa. This position maintains a thorough working knowledge of and adheres to organizational policies, regulations, and procedures.
Qualifications
- A Bachelor's degree in Social Work, Psychology, or related field required.
- At least one year of experience working with youth/young adults is essential and preferred.

- Exceptional verbal and written communication skills.
- Proficiency in navigating electronic health record systems.
- Consistent and timely documentation.
- A vehicle for the purpose of transporting youth
- A valid driver's license
