Role Description
Partnering with preventive cardiologist physician in creating the comprehensive cardiovascular prevention center of excellence of HUMC. See primary and secondary prevention cardiology patients, treating advanced lipid consults, creating clinical protocols, inpatient and outpatient QI projects, teaching and opportunities for clinical research. This is a full-time on-site role for an Advanced Practice Nurse based in Fort Lee, NJ. The Advanced Practice Nurse will provide comprehensive care, including patient assessment, diagnosis, treatment, and follow-up across various healthcare settings. Responsibilities include collaborating with physicians and healthcare teams, developing and implementing care plans, educating patients, and maintaining detailed clinical documentation. Additionally, the role involves participating in quality improvement initiatives and staying updated on evidence-based practices to ensure optimal patient outcomes.
Qualifications
- Advanced clinical expertise in patient assessment, diagnosis, and care planning
- Proficiency in collaborative care models and interdisciplinary teamwork
- Strong patient education skills and ability to communicate with diverse populations
- Commitment to evidence-based practice and continuous professional development
- Active Advanced Practice Nurse license in the state of New Jersey
- National certification in an advanced practice nursing specialty
- Master’s or Doctoral degree in Nursing
- Experience in a healthcare or clinical setting is preferred
