Job Overview
Gulf Coast Social Services is seeking a dedicated and compassionate Case Manager to join our team at the South Central Louisiana Human Services Authority (SCLHSA) St. Mary Behavioral Health Clinic. The Clinic Case Manager (SPOE- Single Point of Entry) serves as the initial point of contact for individuals seeking behavioral health services. This position is responsible for coordinating access to care by conducting clinical screening, assessing client needs, facilitating referrals, and ensuring timely connection to appropriate behavioral health services. The Clinic Case Manager (SPOE) works collaboratively with clinical and administrative staff to promote efficient access, continuity, and positive client outcomes.
Essential Job Functions:
- Serve as the primary point of contact for all incoming behavioral health referrals, phone calls, walk-ins, and aftercare.
- Complete comprehensive clinical screenings to assess presenting concerns, level of functioning, and determine the most appropriate level of care and services.
- Conduct safety and risk assessments, including suicide, homicide, and other behavioral health risk screenings, when clinically indicated, and follow agency protocols for crisis intervention.
- Coordinate with clinical leadership to staff high-risk complex cases and facilitate appropriate disposition.
- Review referrals and supporting documentation to ensure completeness and that the required documentation is met.
- Schedule initial assessments and aftercare appointments.
- Review hospital discharge and aftercare referrals to ensure timely follow-up and continuity of care.
- Complete weekly zero suicide care pathway check-ins and document all required follow-ups.
- Communicate effectively with referral sources, clients, families, providers, and community partners regarding the referral and intake process.
- Maintain knowledge of community resources, agency programs, payer requirements, and behavioral health regulations.
- Perform similar job-related duties as assigned to support the efficient operation of the Single Point of Entry department.
Knowledge, Skills, and Abilities:
- Bachelor's in the Social Science field required; Master's Degree (or working towards a Master's Degree) in counseling, human services, or a related field is preferred.
- Good verbal, written and interpersonal skills, the ability to represent the Agency in a highly professional manner and to maintain the highest level of confidentiality.
- Good judgment, initiative, dependability, sound decision-making skills, the ability to handle multiple responsibilities, strong organizational and time management skills, and the ability to prioritize work and meet multiple deadlines.
- Ability to work independently on assigned tasks and to work collectively with the administrative team.
- A valid Louisiana driver's license, proof of auto liability insurance, and access to a reliable vehicle in safe operating condition.
- Ability to pass a criminal background check, drug screenings, and a Louisiana motor vehicle record check.
