Per Diem Nurses - Home Care Clients
At Guided Living Senior Home Care, we’re reimagining home care with a client-centered approach, and we’re looking for compassionate, reliable, and experienced Nurses to join our per diem team.

What You’ll Do
- Perform initial evaluations and ongoing assessments for new and existing clients
- Conduct home visits in response to changing care needs
- Collaborate with families and care teams to ensure personalized, high-quality service
- Maintain accurate and timely documentation
- Provide education and support to clients and caregivers as needed
Required Qualifications
- A heart for seniors – you're not just a nurse, you're a compassionate advocate
- Active and unrestricted LPN/RN License (Massachusetts)
- Valid driver’s license and reliable transportation
- Flexibility and autonomy in your schedule; ability to respond to clients when needed
- Organized, dependable, and able to work independently
- At least 6 months of LPN/RN experience (home care or similar setting preferred)
- Geriatric experience is a plus, but not required
- Expected to travel up to 30 miles to service Cape needs
