Full-Time sign on Bonus $5,000.00

Requirements:

• Licensed RN in Massachusetts
• Graduate of an approved school for professional nursing,
• A minimum of one year of recent experience in direct patient care nursing.
• Previous home care experience preferred.
• Experience with documentation in a patient Electronic Medical Record is a plus.

Community Nurse is looking for a full-time RN Case Manager to join the Home Care Nursing Team. 35 hours per week with weekend and holiday rotation.

Primary Job Responsibilities:

• Give skilled comprehensive nursing care to patients 18 and older.
• Assess client status upon initial home visit to identify needs in order to develop comprehensive case management plan.
• Using basic teaching principles, counsel and instruct the patient, family and/or significant other in meeting nursing and related needs.
• Coordinate services through organizing, securing, integrating and modifying the resources necessary to accomplish goals set forth in the plan of care.
• Facilitate transition of patient across the continuum to achieve positive patient outcomes.
• Document in an accurate and timely manner.
• Communicate any changes in patient condition and needs to the care team and patient physician.

Job Type: Full-time

Pay: $38.00 - $43.00 per hour
