Guardian Angel Home Health Agency is seeking a compassionate and experienced Registered Dietitian (RD) to join our growing team serving patients throughout Los Angeles and Ventura Counties.
As a member of our interdisciplinary team, you will provide nutritional assessments, education, and care planning to patients in their homes, helping improve health outcomes and quality of life.
Responsibilities
- Perform comprehensive nutritional assessments for Home Health and Hospice patients.
- Develop individualized nutrition care plans based on patient needs and physician orders.
- Provide nutrition counseling and education to patients, families, and caregivers.
- Collaborate with physicians, nurses, therapists, social workers, and other healthcare professionals.
- Monitor patient progress and revise care plans as needed.
- Complete timely and accurate documentation in accordance with agency policies and regulatory requirements.
- Participate in interdisciplinary team meetings and case conferences.
Qualifications
- Current Registered Dietitian (RD/RDN) credential.
- Active California licensure (if applicable).
- Minimum one year of clinical experience preferred.
- Home Health or Hospice experience preferred.
- Strong assessment, communication, and documentation skills.
- Valid California Driver's License, reliable transportation, and current auto insurance.
- Ability to work independently and manage a flexible schedule.

Pay: $105.00 per hour
