Job Overview
Embark on a critical part-time night shift role as a Licensed Practical Nurse dedicated to providing specialized, professional care for residents in a rehabilitation and long-term care setting. You will be instrumental in the clinical operations, ensuring the accurate execution of the nursing process and serving as a key leader. Your responsibilities include expertly directing and monitoring the Certified Nursing Assistant (CNA) team and maintaining a consistent, high standard of meticulous documentation. Your leadership abilities are crucial for the success of your team and optimal patient care outcomes.
Key Responsibilities
- Ensure the timely and accurate administration of medications for your assigned residents, concurrently completing all required documentation within the electronic health record system.
- Perform all prescribed treatments and diligently document the resident's progress and response in accordance with physician orders and established facility protocols.
- Maintain concise, pertinent, and complete resident care documentation, utilizing the computerized medical record process and proactively assuring the initiation of appropriate care pathway tools.
- Be responsible for receiving and delivering relevant clinical reports, both written and verbal, to appropriate staff members according to departmental protocols.
- Conduct continuous, proper monitoring of residents and their condition status, promptly escalating and reporting any significant changes to the charge nurse, supervisor, and/or physicians in a timely manner.
- Complete pre-reviews of medication administration records to ensure compliance with facility protocols.
- Timely complete all required Minimum Data Set (MDS) documentation and related observations.
- Actively support the CNA team through assisting directly with Activities of Daily Living (ADL) care for residents when necessary.
Requirements
- Successful graduation from an accredited School of Practical Nursing.
- Possession of a current and active Licensed Practical Nurse (LPN) license issued by the New York State Education Department (NYSED).
- Current Basic Life Support (BLS) certification from the American Heart Association (AHA).
- Physical capacity to perform Medium Work (exerting $20$ to $50$ pounds of force occasionally, and/or $10$ to $25$ pounds of force frequently, and/or up to $10$ pounds of force constantly to move objects). The role requires frequent walking, standing, or squatting.
Preferred Qualifications
- One year of recent, relevant clinical experience in a comparable setting is preferred.
