Position Summary
Growing Nests is seeking a mission-driven Care Coordinator / Case Manager to provide Medicaid-compliant PNCC services to high-risk pregnant clients. This role is non-clinical and focuses on holistic support—connecting clients to healthcare, education, social services, and emotional support throughout their prenatal journey.
In addition to direct client services, the Case Manager will also supervise PNCC risk assessments, individualized care plans, and ongoing monitoring services to ensure all documentation and services meet DHS and Medicaid standards. The Case Manager provides leadership to other PNCC staff, ensuring quality, accuracy, and consistency of care coordination activities.
About the Program
The Prenatal Care Coordination (PNCC) program is a benefit under Wisconsin Medicaid and BadgerCare Plus. PNCC is designed to reduce adverse pregnancy outcomes by providing high-risk pregnant women with early, continuous, and personalized support.
PNCC services include:
- Risk-based outreach
- Comprehensive assessment and scoring
- Individualized care planning
- Ongoing monitoring and service coordination
- Health education and resource navigation
The program targets key outcomes like tobacco/alcohol cessation, breastfeeding initiation, safe sleep, family planning, mental health support, and paternal involvement.
Primary Responsibilities
- Conduct and supervise comprehensive risk assessments to determine client eligibility and needs.
- Oversee the development and implementation of individualized care plans, ensuring compliance with DHS requirements.
- Supervise ongoing monitoring and care coordination services to confirm accuracy, timeliness, and Medicaid compliance.
- Provide supportive counseling and education on prenatal wellness, safe infant care, family planning, and mental health.
- Deliver health education on prenatal care, breastfeeding, safe sleep, and chronic condition management.
- Provide basic nutrition counseling including maternal diet, infant feeding, and referrals to WIC or community nutrition programs.
- Review and audit PNCC documentation to ensure compliance with Medicaid standards.
- Mentor and guide PNCC staff, offering feedback and professional development opportunities.
- Collaborate with a multidisciplinary team including nurses, dietitians, doulas, and program managers.
- Participate in quality improvement initiatives and assist in program evaluation.
Skills
- Proven experience in crisis intervention and crisis management techniques that effectively de-escalate tense situations.
- Strong background in addiction counseling and behavioral health support tailored to diverse populations.
- Knowledge of child welfare systems, Section 8 housing programs, Medicare regulations, and workforce development initiatives.
- Excellent communication skills with the ability to motivate clients through motivational interviewing techniques.
- Demonstrated supervising experience with the ability to mentor team members or interns in social work settings.
- Familiarity with providing childcare support or mentoring children within social service frameworks.
- Ability to handle sensitive information with confidentiality while maintaining a transparent approach to client care.
- Skilled in developing collaborative relationships with community agencies and service providers to optimize resource access. Preferred Qualifications
- Previous experience in WIC, PNCC, Early Head Start, home visiting, or Medicaid case management.
- Knowledge of community-based maternal and child health systems in Wisconsin.
- Familiarity with trauma-informed care and motivational interviewing approaches.
