About the Role:
As our Social Services Director, you’ll be a key advocate for our residents, helping to ensure their emotional, social, and psychological well-being. This is more than just a job — it's an opportunity to be a trusted support system for residents, families, and staff alike.
In this full-time position (Monday–Friday, 8:00 AM–4:30 PM, with rotating weekends), you’ll:
- Work closely with residents and their families to support emotional, behavioral, and social needs
- Coordinate admissions and discharge planning with empathy and organization
- Conduct in-depth psychosocial assessments and help develop individualized care plans
- Track behavioral and mental health needs and ensure appropriate interventions
- Facilitate family meetings, provide crisis support, and assist with adjustment challenges
- Collaborate with interdisciplinary teams (nursing, activities, dietary, and administration) to provide holistic care
- Ensure compliance with CMS, HIPAA, and state-specific guidelines
- Stay up to date with evolving healthcare regulations and social services best practices
- Connect residents and families with community resources like financial aid, counseling, or legal support
- Champion residents’ rights, dignity, and privacy — always
- Lead by example, supporting, training, and mentoring support staff when applicable
What We’re Looking For:
We’re seeking an experienced, empathetic, and highly organized professional with:
- Social Services experience in a Long-Term Care (LTC) setting ( Preferred )
- Strong knowledge of healthcare regulations (CMS, HIPAA, state guidelines)
- Direct experience supporting elderly individuals with dementia, Alzheimer’s, and other cognitive or physical impairment
- Excellent communication and documentation skills
- Proven ability to assess psychosocial needs and create effective care plans
- A team-first mindset and the ability to advocate for residents in multidisciplinary setting
- Experience handling crises, guiding difficult family transitions, and navigating emotional situations with professionalism and heart
- Passion for working with seniors and helping them maintain dignity, connection, and quality of life
Leadership experience or a background in managing social services staff is a plus!
