We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join a skilled nursing and rehabilitation facility in Parkersburg, WV. In this per-diem role, you'll provide direct patient care, support rehabilitation efforts, and collaborate with an interdisciplinary healthcare team.

• Perform patient assessments and monitor vital signs
• Administer prescribed medications and treatments
• Assist with clinical procedures such as wound care and catheterization
• Collaborate with RNs, physicians, and therapists to implement care plans
• Accurately document patient progress and report changes
• Promote rehabilitation goals through compassionate care
• Ensure adherence to infection control protocols and facility standards

• Education: Graduate of an accredited LPN program
• Licensure/Certifications:
• Active West Virginia LPN license
• Current BLS certification
• Experience:
• At least 1 year of experience in skilled nursing, rehabilitation, or long-term care
• Technical Skills:
• Proficient in medication administration, wound care, and EHR documentation
• Soft Skills:
• Strong communication, empathy, and adaptability in fast-paced environments
