Position OverviewGreenlife Healthcare Staffing is seeking a dedicated Licensed Practical Nurse (LPN) for a per-diem role at a Skilled Nursing and Rehab facility in Decatur, AL. The LPN will play a key role in patient care delivery, focusing on rehabilitation, recovery, and patient/family education in a supportive clinical environment.
Key Responsibilities- Administer medications, treatments, and wound care under RN supervision
- Monitor and document vital signs, patient conditions, and progress
- Collaborate with RNs, physicians, and therapists on patient care plans
- Educate patients and families on recovery goals, medication use, and post-treatment care
- Maintain accurate documentation, including medication administration and shift reports
- Follow all infection control protocols, safety standards, and facility procedures
- Provide emotional support and advocate for patients during their rehab journey

Qualifications- Education: Graduate of an accredited Licensed Practical Nurse program
- Licensure/Certifications:
- Active Alabama LPN license
- BLS and CPR certification (required)
- Experience: At least 1 year in rehabilitation, long-term care, or acute care (preferred)
- Technical Skills: Familiarity with EMR systems and clinical documentation
- Soft Skills: Strong communication, empathy, teamwork, and attention to detail
