Responsibilities for Social Services Director/Activity Director
- Develop and maintain relationships with residents, family members, and caregivers to understand their needs and preferences
- Identify and address any issues or concerns related to our activity program or residents' care
- Participate in interdisciplinary team meetings to share information and collaborate on resident care
- Stay up-to-date with industry trends and best practices in activity programming and social work
- Complete monthly activity calendars
- Help assist resident and family with a safe discharge
- Represent Greenfield Skilled Nursing and Rehabilitation at community events and conferences
Requirements for Social Services Designee/Activity Director
- Develop and maintain relationships with residents, family members, and caregivers to understand their needs and preferences
- Identify and address any issues or concerns related to our activity program or residents' care
- Participate in interdisciplinary team meetings to share information and collaborate on resident care
