Greenfield Rehabilitation Agency is seeking an exceptional Occupational Therapist (OTR) or Certified Occupational Therapy Assistant (COTA) to serve as Rehab Director at our senior living community and skilled nursing facility in Galesville, Wisconsin.

As Rehab Director, You Will:

• Lead and support an interdisciplinary therapy team

• Build and sustain a resident-centered therapy culture

• Mentor clinicians to deliver high-quality, evidence-based care

• Drive clinical excellence, compliance, and operational efficiency

• Foster strong relationships with residents, families, and facility partners

• Oversee scheduling, staffing, productivity, and documentation standards

• Ensure regulatory readiness and adherence to state and federal guidelines

• Promote a positive, accountable, team-oriented environment

Qualifications

• Licensed OTR or COTA in the state of Wisconsin

• Leadership or supervisory experience in senior care or SNF preferred

• Strong communication, organization, and team-building skills
