Greenfield Rehabilitation Agency is seeking a passionate and experienced clinician to join our exceptional therapy team serving a dynamic senior campus that includes short-term rehabilitation, long-term care, assisted living, and independent living. This setting is ideal for a clinician who enjoys clinical variety, interdisciplinary collaboration, and the opportunity to practice at the top of their license.
You will work with residents across multiple levels of care, supporting functional independence, safety, and quality of life. This role is well suited for someone confident managing medical complexity, changing functional status, and fast-paced transitions of care while maintaining a patient-centered approach.

Qualifications:
- Graduate of an accredited school with an associate degree as an Occupational Therapy Assistant.
- Certification by the State of Wisconsin to practice as an Occupational Therapy Assistant
and/or
- Graduate of an accredited school with a bachelor’s, master’s, or doctoral degree in Occupational Therapy.
- License as required by the State of Wisconsin to practice as an Occupational Therapist.
