What are the responsibilities and job description for the Certfied Medical Assistant - SNFology position at Greene County General Hospital?
Under the direction of the RHC Manager, the SNF Certified Medical Assistant is responsible for assisting in the delivery of health care and patient care management for long-term care patients. Ensures effective communication about patient care needs with SNFology providers and long-term care facilities. Primary point of contact with SNFology providers for scheduling and administrative needs.
Essential Duties and Responsibilities:
- Perform administrative duties such as answering phones, scheduling appointments, and maintaining medical records.
- Complete patient registration, intake documentation, and scheduling for daily appointments.
- Scan, fax and upload required documentation into patients’ medical records, including completed chart notes sent to long-term care facilities
- Complete medication reconciliation and process prescription refill requests.
- Facilitate effective communication between providers and long-term care facilities regarding patient care needs.
- Triage and respond to patient communications within practice policy.
- Track monthly provider rounding schedules across all facilities.
- Track new admissions to SNFology practice and maintain ongoing communication with facilities regarding admission status and missing documentation.
- Review incoming faxes and notify providers promptly of urgent matters
- Facilitate effective communication between SNFology team members and program supervisors regarding patient care needs.
- Verify insurance as necessary for billing and administrative purposes.
- Review and respond to correspondence for practice/providers
- Coordinate pharmacy requests for durable medical equipment (DME), including documentation updates, provider signatures, and required paperwork.
- Process medication prior authorizations.
- Communicate with provider/staff on daily via Microsoft Teams to coordinate patient care, appointments and workflow.
- Perform selected patient care and administrative duties and assist the provider as part of the clinical care team
- Coordinate care with disease management or case management programs as appropriate.
- Assist with scheduling tests and treatments, obtaining pre-certification if necessary, for hospital care and/or procedures for patients, and process referrals to other providers.
- Participate in professional development activities and maintain relevant professional affiliations.

Job Qualification Requirements:
Education: Graduate of an accredited Medical Assistant Program
Licensure/Certification: Current Medical Assistant Certification required.(CMA, RMA, NCMA, CCMA or CMAA). CPR certification required within one month of hire.
Experience: One to three years of clinical experience preferred. Long term care experience preferred. Ability to maintain strict confidentiality. Knowledge of Microsoft Word and Excel preferred. Knowledge of admissions protocols and procedures within long term care/SNF environment preferred.
Physical Requirements: Frequent sitting and occasional periods of standing/walking. Long periods of data entry. Occasional bending, stooping, and reaching. Occasionally lift up to 25 lbs.

Schedule: Full Time, Days, Monday- Friday 7:30-4pm
