Job Summary

Responsibilities
- Provide direct patient care, including monitoring vital signs and assisting with daily living activities.
- Administer medications and treatments as prescribed by healthcare providers.
- Conduct patient assessments and document medical histories accurately in electronic health records (EHR).
- Collaborate with healthcare teams to develop and implement individualized care plans.
- Maintain a clean and safe environment for patients by following infection control guidelines.
Skills
- Proficient in using PCC.
- Strong understanding of patient assessment techniques and vital sign monitoring.
- Experience working with populations including geriatrics, Alzheimer's care, memory care, and hospice care.
- Ability to perform diagnostic evaluations and manage acute pain effectively.
- Excellent communication skills for patient education and collaboration with families.
