Responsibilities:
Provide direct patient care to residents, including administering medications, performing wound care, and implementing treatment plans
Conduct initial and ongoing assessments of residents' physical, emotional, and social needs
Develop and implement personalized care plans in collaboration with the interdisciplinary team
Monitor residents' conditions and report changes to the Charge Nurse or other healthcare professionals
Maintain accurate and up-to-date records of resident care and treatment
Participate in quality improvement initiatives and performance improvement activities
Collaborate with other healthcare professionals to ensure seamless coordination of care
Provide emotional support and education to residents and their families
